BONE GRAFTING  

When the shape or size of the upper or lower jawbone is too small to allow placement of a dental implant, bone grafting is frequently recommended to reconstruct and reshape the ridge and gums to accept the dental implant.

The procedure itself is transferring bone from one area of the body(hip, leg, other area) to the deficient area of the jawbone (autogenous or autograft).

Other bone grafting materials are now available which allow bone augmentation without harvesting bone from other parts of the body.

             TYPES OF BONE GRAFTS

  1. Allograft (Bone from other people which has been tissue banked and sterilized)
  2. Xenograft (Bone from animals which has been tissue banked and sterilized)
  3. Recombinant Human Bone Morphogenic Protein (rhBMP-2) produced synthetically in the laboratory.

rhBMP-2 actually stimulates a person’s cells to produce bone.  This material is called INFUSE and is produced by Medtronic Corporation.  It is revolutionizing Maxillofacial bone replacement and bone grafting of the jaws.

It is particularly useful for bone augmentation and grafting for dental implants in tooth replacement as well as for overdentures.

From a cellular standpoint, INFUSE:

  1. Is placed where new bone growth is needed
  2. Attracts your body’s own bone building stem cells to the bone deficient site and over time, new bone is formed.
  3. Bon actually grows where INFUSE bone graft is placed.

As a result of INFUSE  being available for patient bone augmentation, it has resulted in predictable outcomes for patients and has made the placement of dental implants possible in areas of the mouth that previously were not possible.

 Major and minor bone grafting
Over a period of time, the jawbone associated with missing teeth atrophies or is reabsorbed. This often leaves a condition in which there is poor quality and quantity of bone suitable for placement of dental implants. In these situations, most patients are not candidates for placement of dental implants.

We now have the ability to grow bone where needed. This not only gives us the opportunity to place implants of proper length and width, it also gives us a chance to restore functionality and aesthetic appearance. The bone graft procedures are referred to as:

Sinus lift procedure
This procedure involves elevating the sinus membrane and placing the bone graft onto the sinus floor, allowing implants to be placed in the back part of the upper jaw.

Ridge-augmentation
In severe cases the ridge has been reabsorbed and a bone graft is placed to increase the ridge height and/or width.

Nerve- repositioning
The inferior alveolar nerve, which gives feeling to the lower lip and chin, may need to be moved in order to make room for placement of dental implants to the lower jaw.

These procedures may be performed separately or together, depending upon the individual's condition. There are several areas of the body which are suitable for attaining bone grafts. In the maxillofacial region, bone grafts can be taken from inside the mouth, in the area of the chin or third molar region or in the upper jaw behind the last tooth. In more extensive situations, a greater quantity of bone can be attained from the hip or the outer aspect of the tibia at the knee.

These surgeries are performed in the out-patient surgical suite under IV sedation or general anesthesia. After discharge, bed rest is recommended for one day and limited physical activity for one week.
